Physical Examination
Athletes must have a current sports qualifying physical exam on file in the Athletics Office. A separate signed physical form is required for each school year and is valid from May 1 of the current year through June 30 of the succeeding year. Download a physical form HERE.
Once you have your physical completed by a doctor or nurse practioner, you may turn it in to the office or you can scan it and submit the completed Physical Forms online.
